There are three tiers: Essentials at $89 per employee per month (concierge telehealth), Concierge Care at $175, and Concierge Elite at $325. Family add-ons are $99/month for a spouse or partner and $49/month per dependent child.
Yes. Tiers can run side by side across the same company, and family members can be added to any tier. Employee counts and add-ons can be reviewed and adjusted monthly.
No. This membership is not insurance and does not replace major medical, emergency, hospital, surgical, specialist, or imaging coverage. For emergencies call 911. It complements your existing benefits with faster, more personal access to care.
Corporate memberships require a 6-month initial commitment, then continue month-to-month. Either party may discontinue with 30 days written notice. Fees are billed monthly, and employee counts and family add-ons can be adjusted monthly.
We accept ACH (no fees), debit, and all major credit cards (3.5% service fee on cards). Your plan starts on the date you choose and bills on that day each month.
