There are two plans: Concierge Care at $175 per employee per month and Concierge Elite at $325 per employee per month. Household add-ons are $49/month per child under 18 and $99/month per adult 18 and older in the household.
Yes. Both plans can run side by side across the same company, and household members can be added to either plan. Employee counts and add-ons can be reviewed and adjusted monthly.
No. This membership is not insurance and does not replace major medical, emergency, hospital, surgical, specialist, or imaging coverage. For emergencies call 911. It complements your existing benefits with faster, more personal access to care.
Corporate memberships require a 6-month initial commitment, then continue month-to-month. Either party may discontinue with 30 days written notice. Fees are billed monthly, and employee counts and household add-ons can be adjusted monthly.
We accept ACH (no fees), debit, and all major credit cards (3.5% service fee on cards). Your plan starts on the date you choose and bills on that day each month.
